First Visit

During this important visit, we will address your orthodontic needs and determine if it is the proper time to begin treatment. We will help you get acquainted with our office and learn about orthodontic treatment. It is important to understand that each patient’s orthodontic treatment is different. At this first visit, we may proceed with diagnostic records that will help explain all aspects of your treatment in detail. Diagnostic records will help complete a detailed plan that outlines your treatment, goals and estimated fees.

This is a learning experience and we encourage questions from the patient and/or parents. It is our goal and highest priority to educate you about your treatment.

Please complete the patient acquaintance, health history, orthodontic insurance information and HIPPA forms, which can be downloaded and filled out before your arrive.
